I am fully aware of the consequences and dangers involved. In consideration of the club’s granting this membership, i undertake to save harmless the Kelab my-KART Selangor and their officials, servants, agent from and against any actions, claims, costs, expenses and demands in respect of death, injury and losses as the case may be, arising out of or in connection with any entry or my taking part in any meeting and not withstanding that the same may have been contributed to or occassioned by the negligence of the said body(s), their officials, servants, representatives or agents.

For Underage applicants (age of 17 years and below)
I, the undersign, as the Father/Mother/Legal Guardian do hereby give permission to my child/ward to be involved in the club’s events and activities.
